Heart Rate Variability in Patients Developing Atrial Fibrillation in the Course of Acute Myocardial Infarction

The relationship between atrial fibrillation recurrence and autonomic tone or autonomic tone changes preceeding atrial fibrillation attack in paroxysmal forms have been demonstrated in some clinical studies. Atrial fibrillation which develops in the course of acute myocardial infarction is rather complicated issue. In this study, it is sought that, the role of autonomic tone in the development of atrial fibrillation during acute myocardial infarction. Ninety patients (mean age 61.7 ± 11.3 years, 70 male, 20 female) with acute ST elevation myocardial infarction were enrolled to the study. Atrial fibrillation has developed in 9 patients during the the hospital course. Historical evaluation, physical and echocardiographic examinations were done in all patients. Five minutes rest electrograms were obtained in the first 24-hour after admission to the hospital. Afterwards, spectral heart rate variability analysis were performed in these records. Nine patients (mean age 70.9 ± 12.7 years, 8 male, 1 female) with atrial fibrillation were clasiffied as AF(+) group, and 81 patients (mean age 56.1 ± 10.2 years, 62 male, 19 female) were classifed as AF (-) group. Only age was found to be statistically significantly different with an odds ratio of 1.15 (95% CI 1.04-1.27) between the groups. No other demographic, historical or echocardiograhic parameter were found to be different between the groups. Very low frequency and low frequency heart rate variability values were found to be higher in patients with atrial fibrillation when compared to without atrial fibrillation (respectively, 1120.8 ± 656.7 ms2 and 696.5 ± 336.1 ms2 vs. 519.3 ± 905.6 ms2 ve 422.2 ± 797.7 ms2, p>0.05). Although, high frequency values were found to be lower in patients with atrial fibrillation when compared to without atrial fibrillation (respectively, 290.1 ± 98.3 ms2 vs. 655.9 ± 137.1 ms2, p>0.05). As a marker of sympatovagal balance, low frequency, high frequency ratio was found to be slightly higher patients having atrial fibrillation (1.92 ± 1.89’ vs. 1.43 ± 1.47, p>0.05). But, none of the heart rate variability values were statisticaly different between groups. In conclusion, this study showed that age is the main determinator of the development of atrial fibrillation during the course of acute myocardial infarction. Despite heart rate variability parameters have a slight dominans in favour of sympathetic activity in atrial fibrillation group, this difference did not reach any statistical significance.
