1.The impact of diabetes and coronary artery disease on mortality and morbidity in patients with hypertension

Amaç: Hipertansif hastalarda diabetes mellitus (DM) ve/veya koroner arter hastalığının (KAH) kardiyovasküler sonlanım noktaları üzerine etkisi değerlendirildi.
Ça­lış­ma pla­nı: Kesitsel, çokmerkezli, gözlemsel bir çalışma olan Vasküler Risk Çalışması, Türkiye’nin farklı bölgelerinden 5600 hasta ile yürütülmektedir. Bu çalışma grubunda yer alan hipertansiyonlu 4506 hastadan takipleri yapılan 2664’ü (1643 kadın, 1021 erkek; ortalama yaş: 65.3; dağılım 55-99) değerlendirildi. Beş yıllık takip sonundaki kardiyovasküler birincil ve ikincil sonlanım noktaları, sadece hipertansiyon bulunan hastalarda ve DM ve/veya KAH bulunan hastalarda araştırıldı. Ölüm nedeniyle ilgili bilgi hastaların yakınlarından takip telefonlarıyla elde edildi.
Bul­gu­lar: İncelenen hastaların 1171’inde (%44) sadece hipertansiyon, 631’inde (%23.7) DM, 530’unda (%19.9) KAH, 332’sinde (%12.5) DM ve KAH vardı. Diyabet ve KAH’den birinin olduğu hastalarda tüm sonlanım noktalarında anlamlı artış görüldü. Birincil ve ikincil sonlanım noktaları, kardiyovasküler ölüm ve tüm nedenli ölümlerin oranları, hipertansiyon yanında sadece DM olan ve sadece KAH olan hastalarda benzer bulundu. Sağkalım analizinde, en düşük kardiyovasküler ölüm oranı sadece hipertansiyon olan hastalarda (%5.7), en yüksek oran ise hem DM hem de KAH olan hastalarda (%18.4) görüldü. Hipertansif grupta sadece DM ve sadece KAH olan hastalarda kümülatif sağkalım oranları benzer bulundu (p>0.05).
So­nuç: Bu çalışmada, hipertansif hastalarda DM varlığının oluşturduğu kardiyovasküler riskin, KAH varlığının oluşturduğu riske eşit olduğu ve bunların ikisinin bir arada bulunduğu hipertansif hastalarda riskin geometrik olarak arttığı gösterilmiştir.
Objectives: We evaluated the impact of diabetes mellitus (DM) and/or coronary artery disease (CAD) on cardiovascular endpoints in a cohort of hypertensive patients.
Study design: The Vascular Risk Study is a cross-sectional, multicenter, observational study conducted among 5,600 patients from various regions of Turkey. This analysis included 2,664 patients (1,643 women, 1,021 men; mean age 65.3 years; range 55-99 years) whose follow-up data were adequate among a population of 4,506 hypertensive subjects. Cardiovascular primary and secondary endpoints at the end of a five-year follow-up were assessed in patients who had hypertension alone, and in those having DM and/or CAD. Information on the cause of death was obtained from the relatives of the patients by follow-up phone calls.
Results: There were 1,171 patients (44%) with isolated hypertension, 631 (23.7%) with DM, 530 (19.9%) with CAD, and 332 (12.5%) with both DM and CAD. The presence of either DM or CAD was associated with significant increases in the incidences of all endpoints. The occurrences of primary and secondary endpoints, cardiovascular death, and all death were similar in hypertensive patients who had DM without CAD and in patients who had CAD without DM. In survival analysis, the incidence of cardiovascular death was lowest (5.7%) in hypertensive patients without DM and CAD, and highest (18.4%) in hypertensive patients with DM and CAD. The cumulative survival rates were similar in hypertensive patients with either DM or CAD alone (p>0.05).
Conclusion: This study demonstrated that the level of cardiovascular risk associated with DM was equal to the risk associated with CAD in hypertensive patients and that the coexistence of DM and CAD in these patients increases the risk geometrically.

2.Assessment of autonomic dysfunction and anxiety levels in patients with mitral valve prolapse

Amaç: Bu çalışmada mitral kapak prolapsı (MKP) olan hastalarda otonomik fonksiyon bozuklukları ve anksiyete düzeyleri değerlendirildi.
Ça­lış­ma pla­nı: Çalışmaya MKP tanısı konan 33 hasta (ort. yaş 25±5) ve 14 sağlıklı kişi (ort. yaş 25±4) alındı. Hastalar kapakçık kalınlığında anormal artış (>5 mm) olup (anatomik MKP, n=11) olmamasına (MKP sendromu, n=22) göre iki gruba ayrıldı. Hasta ve kontrol gruplarına Spielberger Durumluluk Anksiyete Ölçeği (SDAÖ) ve Spielberger Süreklilik Anksiyete Ölçeği (SSAÖ) uygulandı ve kalp hızı (KH) ve arteryel kan basınçları (KB) sırtüstü yatış ve ayakta pozisyonlarında ölçüldü.
Bul­gu­lar: Anatomik MKP’li hastalarda fizik muayenede orta sistolik klik ve geç sistolik üfürüm daha sık görülürken, MKP sendromlu hastalarda nefes darlığı, vertigo, atipik göğüs ağrısı gibi spesifik olmayan semptomlar daha yüksek oranlarda görüldü (p<0.05). Mitral yetersizliği (orta derecede) anatomik MKP grubunda anlamlı derecede daha yüksek oranda görüldü (%72.7 ve %22.7; p<0.009). Ortalama SDAÖ ve SSAÖ skorları MKP sendromlu hastalarda (41.0±15.6 ve 38.5±15.5) anatomik MKP grubundakilere (15.8±7.5 ve 17.0±9.1) ve kontrollere (14.9±7.4 ve 16.9±8.7) göre anlamlı derecede yüksekti (p<0.001). Ortostatik KB ve KH farklılıkları MKP sendromlu grupta anatomik hastalığı olanlara göre anlamlı derecede fazlaydı (sırasıyla, p<0.001 ve p=0.032). Ortostatik KH farklılıkları her iki MKP grubunda SDAÖ ile anlamlı ilişki gösterirken (r=0.536, p=0.001), SSAÖ ile sadece MKP sendromlu grupta anlamlı ilişkiliydi (r=0.523, p=0.002). Ortostatik KB farklılıkları her iki hasta grubunda da aksiyete parametreleri ile negatif ilişkili bulundu (SDAÖ için, r=-0.391, p=0.025; SSAÖ için r=-0.320, p=0.048).
So­nuç: Bulgularımız, otonomik fonksiyon bozukluğu ve anksiyete göstergelerinin MKP sendromlu hastalarda anatomik MKP’li olgulara göre daha yüksek olduğunu göstermektedir.
Objectives: This study aimed to assess autonomic dysfunction parameters and anxiety levels in patients with mitral valve prolapse (MVP).
Study design: We evaluated 33 patients (mean age 25±5 years) with MVP and 14 healthy subjects (mean age 25±4 years). The patients were divided into two groups according to the presence (anatomical MVP, n=11) or absence (MVP syndrome, n=22) of abnormal leaflet thickening (>5 mm). Spielberger’s Situational Anxiety Scale (SSAS) and Continuous Anxiety Scale (SCAS) were administered to all the subjects, and heart rates (HR) and arterial blood pressures (BP) were measured in the supine and standing positions.
Results: Mid-systolic click and late systolic murmur were significantly more frequent in patients with anatomical MVP, while nonspecific symptoms such as dyspnea, vertigo, and atypical chest pain were more frequent in patients with MVP syndrome (p<0.05). Mitral insufficiency (mild) was significantly more frequent in patients with anatomical MVP (72.7% vs. 22.7%; p<0.009). Patients with MVP syndrome had significantly higher SSAS and SCAS scores (41.0±15.6 and 38.5±15.5) compared to patients with anatomical MVP (15.8±7.5 and 17.0±9.1) and controls (14.9±7.4 and 16.9±8.7, respectively; for both p<0.001). Orthostatic differences in BP and HR were significantly greater in patients with MVP syndrome than those having anatomical MVP (p<0.001 and p=0.032, respectively). Orthostatic HR differences showed a significant correlation with SSAS in both MVP groups (r=0.536, p=0.001) and a significant correlation with SCAS in patients with MVP syndrome (r=0.523, p=0.002). There was an inverse correlation between orthostatic BP differences and anxiety parameters in all MVP patients (r=-0.391, p=0.025 for SSAS, and r=-0.320, p=0.048 for SCAS).
Conclusion: Our data suggest that patients with MVP syndrome have increased autonomic dysfunction and anxiety scores compared to patients with anatomical MVP.

3.Mentholated cigarette smoking and brachial artery, carotid artery, and aortic vascular function

Amaç: Bu çalışmada, başka açıdan sağlık sorunu olmayan gençlerde mentollü ve mentollü olmayan sigara kullanımının vasküler fonksiyonlara ve sol ventrikül diyastolik fonksiyonlara etkisi araştırıldı.
Ça­lış­ma pla­nı: Çalışmaya sigara içen 20 (6 kadın, 14 erkek; ort. yaş 25.6) ve sigara kullanmayan 22 (12 kadın, 10 erkek; ort. yaş 25.1) sağlıklı kişi alındı. Deneklerin başlangıç özelliklerini belirlemek için ultrason ve ekokardiyografik incelemeler yapıldı; brakiyal arter, aort ve karotis arter incelemelerinden brakiyal akım aracılı genişleme (FMD), aort ve karotis arterler için sertleşme indeksi (Sİ), distensibilite ve elastik modulus (EM) ölçüldü. İkinci gün, her bir gönüllüye iki adet mentollü ya da mentolsüz sigara içirilerek ultrason ve ekokardiyografik incelemeler tekrarlandı. On beş gün sonra, her bir deneğe ilk ölçümde kullanılan sigara yerine diğeri içirilerek ölçümler tekrarlandı.
Bul­gu­lar: Mentollü ve mentolsüz sigara içilmesinden sonra FMD değerleri %14.0±9.0’dan sırasıyla %8.3±3.2 (p=0.012) ve %9.8±5.5’e (p=0.025) geriledi. Sistolik kan basıncında anlamlı artış sadece mentollü sigara içimiyle görüldü (p=0.003). Kalp hızı ve hız-basınç ürünü değerleri, mentollü sigarayla daha belirgin olmak üzere, iki grupta da anlamlı artış gösterdi. İki tür sigarayla da, mitral E yavaşlama zamanında anlamlı artış ve mitral E/A oranında anlamlı düşüş gözlendi. Aorta ait Sİ ve EM değerleri sadece mentollü sigara kullanımı sonrası anlamlı değişim gösterirken, karotis Sİ ve EM değerleri iki tür sigara ile de anlamlı değişim gösterdi. Sigara içiminden sonraki değerlerin karşılaştırılmasında, mentollü sigara kullanımında, sistolik kan basıncı, kalp hızı, hız-basınç ürünü, karotis gerinimi ve karotis Sİ parametrelerinde, mentolsüz sigara içimine göre anlamlı farklılık saptandı (sırasıyla, p=0.027, p<0.001, p<0.001, p=0.037 ve p<0.001).
So­nuç: Bulgularımız mentollü sigaranın mentolsüz sigaradan daha az zararlı olmadığını; hatta, mentollü sigaranın elastisite ve sertlikle ilgili parametrelere daha ciddi akut zararlar verdiğini göstermektedir.
Objectives: We investigated possible acute effects of mentholated versus nonmentholated cigarette smoking on vascular functions and left ventricular diastolic functions in otherwise healthy young smokers.
Study design: The study included 20 otherwise healthy smokers (6 women, 14 men; mean age 25.6 years) and 22 healthy nonsmokers (12 women, 10 men; mean age 25.1 years). Ultrasound and echocardiographic examinations were performed to determine baseline characteristics for the brachial artery, aorta, and carotid artery, including brachial flow-mediated dilation (FMD), aortic and carotid stiffness index (SI), distensibility, and elastic modulus (EM). On day 2, each subject smoked either two mentholated or nonmentholated cigarettes and ultrasound and echocardiographic examinations were repeated. The procedure was repeated 15 days later with each subject smoking the other type of cigarette.
Results: From the baseline level of 14.0±9.0%, FMD decreased significantly to 8.3±3.2% (p=0.012) and to 9.8±5.5% (p=0.025) after smoking mentholated and nonmentholated cigarettes, respectively. Increase in systolic blood pressure was significant only with mentholated cigarettes (p=0.003). Increases in heart rate and rate-pressure product were significant in both groups, being more prominent with mentholated cigarettes. Both types of cigarettes resulted in significant prolongation of mitral E deceleration time and decrease in mitral E/A ratio. Changes in aortic SI and EM were significant only with mentholated cigarettes, while changes in carotid SI and EM were significant in both groups. Menthol-associated changes in systolic blood pressure, heart rate, rate-pressure product, carotid strain, and carotid SI parameters differed significantly from those seen after nonmentholated cigarette smoking (p=0.027, p<0.001, p<0.001, p=0.037, and p<0.001, respectively).
Conclusion: Our findings show that mentholated cigarettes are not safer than nonmentholated cigarettes and that menthol-associated acute impairment is more severe in many parameters of elasticity and stiffness.

4.The association between serum adiponectin levels and the severity of coronary artery lesions on the angiogram

Amaç: Koroner arter hastalığı (KAH) olanlarda serum adiponektin düzeylerinin düştüğü gösterilmiştir. Bu çalışmada, serum adiponektin düzeyleri ile anjiyogramda koroner lezyon ciddiyeti arasındaki ilişki değerlendirildi.
Ça­lış­ma pla­nı: Çalışmaya koroner anjiyografide koroner arterlerinde ciddi lezyon (≥%50 darlık) saptanan 86 hasta (70 erkek, 16 kadın; ort. yaş 60) alındı. Koroner lezyonlu damar sayısına göre hastalar üç gruba ayrıldı; 18 hastada tek damar, 16 hastada 2 damar, 52 hastada çok damar hastalığı vardı. Koroner lezyonların ciddiyeti modifiye Gensini skoru ile değerlendirildi. Serum adiponektin düzeyleri hasta grubunda ve anjiyografide koroner arterleri normal bulunan 33 kişiden (16 erkek, 17 kadın; ort. yaş 54.8) oluşan kontrol grubunda ölçüldü ve karşılaştırıldı.
Bul­gu­lar: Hasta grubunda ortalama yaş, erkek hastaların ve sigara içenlerin sayısı kontrol grubundan anlamlı olarak daha yüksekti (p=0.01). Serum adiponektin düzeyleri hasta grubunda (2.0±2.0 µgr/dl) kontrol grubuna (3.2±2.7 µgr/dl) göre anlamlı derecede düşük bulunurken (p=0.01), lezyonlu damar sayısına göre altgruplar arasında serum adiponektin düzeyleri anlamlı farklılık göstermedi. Serum adiponektin düzeyi tek damar lezyonlu grupta (2.6±2.5 µgr/dl) kontrol grubundan anlamlı fark göstermedi; iki damar (1.5±0.9 µgr/dl) ve çok damar (2.0±2.0 µgr/dl) lezyonlu gruplarda ise kontrol grubundan anlamlı derecede düşüktü (p=0.01). Hasta grubunda Gensini skoru ortalaması 3.8±1.7 bulundu. Korelasyon analizinde, serum adiponektin düzeyi Gensini skoru ile, zayıf da olsa, anlamlı negatif bir ilişki gösterdi (r=-0.209; p=0.02).
So­nuç: Serum adiponektin düzeyleri, KAH’de kontrollere göre anlamlı düşüş göstermektedir. Koroner lezyon ciddiyeti arttıkça belirginleşen bu düşüş çok damar hastalığı için bir ipucu olabilir.
Objectives: Decreased serum adiponectin levels have been shown in patients with coronary artery disease (CAD). We evaluated the association between serum adiponectin levels and CAD severity on the angiogram.
Study design: The study included 86 patients (70 males, 16 females; mean age 60 years) with angiographically documented CAD (≥50% stenosis). The patients were divided into three groups according to the number of vessels affected; thus, 18 had single-vessel, 16 had two-vessel, and 52 had multiple-vessel disease. The severity of coronary lesions was assessed using the modified Gensini score. Serum adiponectin levels were measured in the CAD group and in a control group of 33 subjects (16 males, 17 females; mean age 54.8 years) who were found to have normal coronary arteries on angiography.
Results: The mean age, the number of male patients, and the number of smokers were significantly higher in the CAD group (p=0.01). Patients with CAD exhibited significantly lower serum levels of adiponectin compared to the control group (2.0±2.0 µmg/dl vs. 3.2±2.7 µmg/dl; p=0.01). There were no significant differences in adiponectin levels between patients with single-, two, and multiple-vessel disease. Compared to the controls, patients with two- and multiple-vessel disease had significantly lower adiponectin levels (1.5±0.9 µg/dl and 2.0±2.0 µg/dl, respectively), whereas those with single-vessel disease (2.6±2.5 µg/dl) did not differ from the controls in this respect. The mean Gensini score was 3.8±1.7 in the CAD group. There was a weak inverse correlation between serum adiponectin levels and the Gensini score (r=-0.209; p=0.02).
Conclusion: Serum adiponectin levels are decreased in CAD patients compared to controls. This decrease is more prominent with increasing levels of CAD severity, which may be a helpful clue of multivessel disease.

5.The effect of transmyocardial laser revascularization on anginal symptoms and clinical results in patients with incomplete surgical revascularization

Amaç: Bu çalışmada, koroner arter baypas cerrahisi (KABC) ile tam revaskülarizasyon sağlanamayan hastalarda transmiyokardiyal lazer revaskülarizasyonu (TMR) uygulamasının anjinal yakınmalar ve klinik sonuçlar üzerindeki etkileri araştırıldı.
Ça­lış­ma pla­nı: 2003-2006 yılları arasında KABC uygulanan ancak tam revaskülarizasyon sağlanamayan 45 hastaya ait veriler geriye dönük olarak incelendi. Bu gruptan 35 hastaya (ort. yaş 61.7) sadece KABC, 10 hastaya (ort. yaş 62) ise KABC ile eşzamanlı olarak TMR işlemi yapılmıştı. Hastalar üçüncü ayda ve ortalama 22.3±6.1 ay olan takip süresinin sonunda transtorasik ekokardiyografi ve perfüzyon sintigrafisi ile incelendi. Anjinal semptomlar CCS (Canadian Cardiovascular Society) sınıflamasına göre değerlendirildi.
Bul­gu­lar: İki grubun ameliyat öncesi verileri benzer bulundu. Hastaların tümü semptomatikti (CCS sınıfı TMR grubunda 2.6±0.5, sadece KABC grubunda 2.3±0.8). Kardiyopulmoner baypas (KPB) süresi TMR grubunda daha uzun idi (p=0.022). Ameliyat sonunda TMR grubunda düşük-orta doz inotrop desteğine daha az ihtiyaç duyuldu (TMR grubunda %10, sadece KABC grubunda %48.6; p=0.034). Erken mortalite hiçbir hastada gözlenmezken, üç hastada (TMR grubunda 1 hasta, sadece KABC grubunda 2 hasta; p=0.329) geç mortalite görüldü. Üçüncü ayda TMR grubundaki hastaların %50’si (n=5) asemptomatik iken, sadece KABC grubunda hastaların %14.3’ü (n=5) asemptomatikti (p=0.016). Anjinal semptomlarda düzelme TMR grubunda anlamlı derece daha fazla idi (CCS sınıfı 3. ay 1.2±0.6 ile 2.2±0.7; p=0,001 ve takip sonu 1±0.6 ile 2±0.7; p=0.001). Miyokart perfüzyon sintigrafisi ve ekokardiyografi ile kontrollerde iki grup arasında anlamlı fark görülmedi.
So­nuç: Tam cerrahi revaskülarizasyon sağlanamayan hastalarda TMR uygulaması KPB’den ayırma işlemi sırasında inotropik destek ihtiyacını, kısa ve orta dönemde anjinal yakınmaları azaltmaktadır.
Objectives: We evaluated the effect of transmyocardial laser revascularization (TMR) on anginal symptoms and clinical results in patients in whom coronary artery bypass grafting (CABG) surgery was not sufficient to provide complete revascularization.
Study design: This retrospective study included 45 patients who underwent CABG surgery with incomplete revascularization between 2003 and 2006. Of these, 35 patients (mean age 61.7 years) had CABG alone, while 10 patients (mean age 62 years) underwent TMR at the same session as an adjunct to CABG. All the patients were assessed by transthoracic echocardiography and myocardial perfusion scintigraphy at three months and after a mean follow-up period of 22.3±6.1 months. Anginal symptoms were assessed using the CCS (Canadian Cardiovascular Society) classification system.
Results: Preoperative variables were similar in both groups. All the patients were symptomatic preoperatively with mean CCS scores of 2.6±0.5 and 2.3±0.8 in the TMR and CABG-alone groups, respectively. The duration of cardiopulmonary bypass (CPB) was significantly longer in the TMR group (p=0.022). During weaning from CPB, the need for inotropic support was significantly less in the TMR group (10% vs. 48.6%; p=0.034). While there was no early mortality, late mortality occurred in three patients (1 in TMR, 2 in CABG-alone groups; p=0.329). At three months, 50% (n=5) of the TMR patients were asymptomatic, compared to 14.3% (n=5) in the CABG-alone group (p=0.016). Patients in the TMR group had significantly lower CCS angina scores at three months (1.2±0.6 vs. 2.2±0.7; p=0.001) and at the end of the follow-up (1±0.6 vs. 2±0.7; p=0.001). There were no significant differences between the two groups with regard to the findings of myocardial perfusion scintigraphy and echocardiography.
Conclusion: Patients with incomplete surgical revascularization benefit from TMR in terms of decreased need for inotropic support during weaning from CPB and short- and mid-term relief of angina symptoms.

6.Idiopathic pulmonary artery aneurysm in a patient presenting with chest pain

Pulmoner arter anevrizması (PAA) nadir görülen bir durumdur. İzole ya da idiyopatik PAA daha da nadirdir. Altmış iki yaşında kadın hasta eforla ortaya çıkan göğüs ağrısı, çarpıntı ve nefes darlığı şikayetleri ile başvurdu. Kan basıncı 100/60 mmHg, radiyal nabız 100 atım/dk ve düzenliydi. Transtorasik ekokardiyografide ana pulmoner arter ve dallarının ileri derecede genişlediği saptandı. Triküspit yetersizliği akımı üzerinden sistolik pulmoner arter basıncı 30 mmHg ölçüldü. Transözofageal ekokardiyografide ana pulmoner arter ve dallarının genişlediği görüldü, trombüs saptanmadı. Çokkesitli bilgisayarlı tomografide ana pulmoner arter 53 mm, sol pulmoner arter 42 mm, sağ pulmoner arter 34 mm ölçüldü. Hastada PAA’ya yol açabilecek herhangi bir neden bulunamadı.
Pulmonary artery aneurysm (PAA) is a rare entity, isolated or idiopathic PAA is even less common. A 62-year-old woman presented with complaints of chest pain on exertion, palpitation, and dyspnea. Her blood pressure was 100/60 mmHg, and radial pulse was 100 beats/min and rhythmic. Transthoracic echocardiography demonstrated extreme dilatation of the main pulmonary artery and its branches. Systolic pulmonary artery pressure measured over the tricuspid regurgitation was 30 mmHg. Transesophageal echocardiography confirmed the presence of dilated pulmonary arteries and there was no thrombus. By multislice computed tomography, the diameters of the main, left, and right pulmonary arteries were measured as 53 mm, 42 mm, and 34 mm, respectively. No cardiac or pulmonary cause was found for the PAA.

7.Persistent atrial standstill and idioventricular rhythm in a patient with thalassemia intermedia

Bu yazıda, 30 yıl önce talasemi intermedia tanısı konan ve sağ kalp yetersizliği gelişen 57 yaşında bir erkek hasta sunuldu. Hastanın kısa süre demir tedavisi gördüğü, ancak hiç kan transfüzyonu yapılmadığı öğrenildi. Hemoglobin düzeyi 7.1 gr/dl, hematokrit 22.7% idi. Beyaz küre ve platelet sayısı ve serum demir düzeyi normaldi. Elektrokardiyogramda, yavaş hızlı atriyal fibrilasyon olarak düşünülen, 35/dk hızda dar QRS kompleksli ritim izlendi. Ekokardiyografide, sağ atriyum ve ventrikül genişlemesi, sağ ventrikül sistolik fonksiyonunda azalma, ileri derece triküspit yetersizliği ve hafif perikardiyal efüzyon saptandı. Elektrofizyolojik çalışmada sağ atriyumda elektriksel aktivite kaydedilemedi. Sağ atriyumun çeşitli bölgelerine yapılan uyarılara yanıt alınamadı ve sağ ventriküler pacing ile sağ atriyuma retrograt iletim gözlenmedi. His bölgesi kayıtlarında, farklı odaklardan köken alan ventriküler aktivitenin retrograt olarak His bölgesini aktive ettiği görüldü. Ventriküler ritim sırasında retrograt V-H iletim zamanı 95 msn ile değişiklik göstermedi. Retrograt nodal iletim yoktu. Hastaya VVIR kalp pili takıldı. Altı aylık takip sırasında hasta kendini iyi hissettiğini belirtti; fonksiyonel durumu NYHA sınıf II idi. Temel ritmi, 20/dk ile genişlemiş QRS aritmisi şeklindeydi. Bildiğimiz kadarıyla, sağ kalple sınırlı perikardiyal efüzyon ve sağ kalp yetersizliğinin eşlik ettiği atriyal elektriksel inaktivite, talasemik bozukluklarda ilk kez bildirilmektedir.
We present a 57-year old male patient with thalassemia intermedia and right heart failure. He had a 30-year history of anemia and short-term iron therapy without blood transfusion. Hemoglobin level was 7.1 g/dl and hematocrit was 22.7%. White blood-cell and platelet counts, and serum ferritin level were normal. Electrocardiography showed irregular narrow QRS bradyarrhythmia, suggesting slow atrial fibrillation at a mean rate of 35 beats/min. Echocardiographic examination revealed dilatation of the right atrium and ventricle, depressed systolic right ventricular function, advanced tricuspid regurgitation, and mild pericardial effusion. In the electrophysiologic study, no electrical activity was recorded in the right atrium. It was inexcitable at multiple sites and no retrograde conduction to the right atrium could be elicited by ventricular pacing. His bundle (HB) recording showed fixed retrograde HB activation with ventricular rhythm originating from different foci. Retrograde V-H conduction time during ventricular rhythm was 95 msec and did not change. There was no retrograde nodal conduction. A VVIR pacemaker was implanted. During a six-month follow-up, he felt well, his functional capacity was NYHA class II, and his basic rhythm was widened QRS arrhythmia with a rate of 20 beats/min. To the best of our knowledge, atrial electrical inactivity together with right-heart failure and pericarditis confined to the right heart chambers has hitherto not been reported in thalassemic disorders.

8.A rare cause of aortic insufficiency: quadricuspid aortic valve

Dört yaprakçıklı aort kapağı, aort yetersizliğinin nadir bir nedeni olarak genellikle tesadüfi bir bulgu şeklinde karşımıza çıkmaktadır. Yirmi dört yaşında asemptomatik bir erkek hastada, rutin muayenede kardiyak üfürüm saptanması üzerine kardiyak değerlendirme yapıldı. Sternumun sağ üst kenarında, 1/6 derecede erken diyastolik üfürüm duyuldu. Elektrokardiyografisi sinüs ritminde ve normaldi. Transtorasik ekokardiyografide sol ventrikül çapları ve sistolik fonksiyonları normal bulunurken, kapak yapısı net değerlendirilemedi. Renkli Doppler ekokardiyografide hafif aort yetersizliği saptandı. Transözofageal ekokardiyografide aort kapağının birbirine eşit büyüklükte iki büyük ve iki küçük yaprakçık içerdiği görüldü. Renkli Doppler çalışmada, küçük ve büyük yaprakçıkların birleştiği her iki bölgeden aort yetersizlik akımları izlendi. Aort bilgisayarlı tomografik (BT) anjiyografi ve kardiyak BT incelemelerinde aort koarktasyonu ya da herhangi bir koroner anomali saptanmazken, dört yaprakçıklı kapak yapısı kardiyak BT ile de doğrulandı.
Quadricuspid aortic valve can be documented incidentally as a rare cause of aortic regurgitation. An asymptomatic 24-year-old male patient underwent cardiac evaluation after detection of a cardiac murmur on routine examination. There was a grade 1/6 early diastolic murmur on the right upper side of the sternum. The electrocardiogram was in sinus rhythm and normal. Transthoracic echocardiography showed normal left ventricular dimensions and functions, but it did not provide an adequate visualization of valve structure. Color Doppler examination showed mild aortic insufficiency. Transesophageal echocardiography revealed a qaudricuspid aortic valve composed of two equal large cusps and two equal small cusps. Color Doppler scans demonstrated aortic regurgitant flow at the coaptation sites of small and large cusps. Further studies by aortic computed tomographic (CT) angiography and cardiac CT did not show aortic coarctation or any other coronary abnormality, but the latter confirmed quadricuspid aortic valve.

9.Mitral valve aneurysm associated with aortic valve regurgitation

Mitral kapak anevrizması nadirdir ve genellikle aort kapağında gelişen enfektif endokardit sonucu ortaya çıkar. Bu yazıda, 66 yaşında bir erkek hastada saptanan mitral kapak ön yaprakçık anevrizması sunuldu. İkiboyutlu transtorasik ekokardiyografi ve transözafageal ekokardiyografide, mitral kapak ön yaprakçığında, tüm kalp siklusu boyunca sol atriyuma bombeleşen sakküler bir yapı, aort kapağında anevrizmal lezyon ve ciddi mitral ve aort yetersizliği saptandı. Vejetasyon veya atriyal trombüs bulgusuna rastlanmadı. Hastanın öyküsünde geçirilmiş enfektif endokardit ile ilgili bir olay yoktu. Mitral ve aort kapaklar mekanik protez kapak ile değiştirildi. Çıkarılan kapakların patolojik incelemesinde enflamasyon izlendi. Örneklerin kültür sonuçları da negatif idi. Ameliyat sonrası dönemi olaysız geçiren hasta beşinci günde taburcu edildi. Hastadaki mitral kapak anevrizmasının, aort kapakta daha önce geçirilmiş enfektif endokardit sekeline bağlı anevrizmal oluşum ve ciddi aort yetersizliğinden kaynaklandığı düşünüldü.
Mitral valve aneurysm (MVA) is uncommon and occurs most commonly in association with infective endocarditis involving the aortic valve. A 66-year-old man with anterior MVA is presented. Two-dimensional transthoracic echocardiography and transesophageal echocardiography revealed a saccular structure in the anterior mitral leaflet that bulged into the left atrium throughout the cardiac cycle, a localized aneurysmal lesion of the aortic valve, and severe mitral and aortic regurgitation. There were neither vegetations nor atrial thrombi and his medical record was not suggestive of any episode of infective endocarditis. The mitral and aortic valves were replaced with mechanical protheses. Pathologic examination of the excised valves showed inflammation and cultures were negative. The postoperative course was uneventful, and the patient was discharged on the fifth postoperative day. In this case, MVA is likely to result from previous infective endocarditis of the aortic valve leading to aneurysm formation and severe aortic regurgitation.

10.A rare cause of reversible dilated cardiomyopathy: hypocalcemia

Hipokalsemi geri dönüşümlü kalp yetersizliğinin nadir bir nedenidir. Bu yazıda, tedaviye dirençli ileri derecede kalp yetersizliği olan 27 yaşındaki bir hasta sunuldu. D vitaminine dirençli raşitizm nedeniyle 20 yaşına kadar D vitamini ve kalsiyum replasman tedavisi gören hastanın son yedi yıldır kalsiyum ve D vitamini tedavisini bıraktığı öğrenildi. Hastada ileri derecede hipokalsemi vardı. Ekokardiyografide sol ventrikül genişlemesi, global hipokinezi (ejeksiyon fraksiyonu %25), üçüncü derece mitral yetersizliği ve ikinci derece triküspit yetersizliği saptandı. Kalsiyum ve D vitamini replasmanı sonrası hastanın semptomları hızla geriledi. Dokuzuncu ayda miyokart disfonksiyonu tamamen düzeldi. Tedaviye dirençli kalp yetersizliğinin nedenleri arasında hipokalsemi de düşünülmelidir.
Hypocalcaemia is a rare cause of reversible heart failure. We present a 27-year-old man who had severe heart failure unresponsive to medical therapy. He had vitamin D-resistant rickets for which he had received replacement therapy with vitamin D and calcium until age 20, but he discontinued treatment for the past seven years. Severe hypocalcemia was detected. Echocardiography showed left ventricular dilatation, global hypokinesia (ejection fraction 25%), and mitral and tricuspid regurgitation of grades 3 and 2, respectively. After calcium and vitamin D supplementation, his symptoms showed rapid improvement. At nine months, myocardial dysfunction improved fully. Hypocalcemia should be considered among the causes of heart failure unresponsive to medical treatment.

11.Left ventricular noncompaction associated with Ebstein’s anomaly

Sol ventrikülde süngerimsi miyokart (noncompaction)endomiyokardiyal morfogenez sırasındaki bir duraklamadan kaynaklanan, belirgin trabeküller ve bu trabeküllerin arasında kalan derin girintilerle karakterize, doğumsal bir kalp hastalığıdır. Ebstein anomalisi ile birlikteliği çok nadirdir. On üç yaşında bir erkek çocuk, sportif bir etkinliğe katılım öncesinde kardiyak açıdan değerlendirildi. Fizik muayenede, triküspit kapaktan gelen 1/6 derecesinde sistolik üfürüm saptandı. Elektrokardiyogramı normaldi. Transtorasik ekokardiyografide, interventriküler septumun apeksinde, yan duvarında ve apikal kısmında çok sayıda büyük trabekül ve derin intratrabeküler girinti görüldü. Renkli akım Doppler incelemesinde trabeküller içinde kan akımı görüldü. Sol ventrikül boyutları ve ejeksiyon fraksiyonu (%65) normaldi. Triküspit kapağın septal ve inferior yaprakçıklarının birleştiği kenarda apikal yer değişikliği izlendi ve hafif triküspit yetersizliği vardı. Mitral ve triküspit halkalar arasındaki mesafe 20 mm ölçüldü. Sağ ventrikülde genişleme yoktu ve sistolik fonksiyonu normal idi; sağ atriyumda ise orta derece genişleme vardı. Hasta asemptomatik olduğundan ve sol ventrikül sistolik fonksiyonu korunmuş olduğundan, düzenli takip programına alındı ve düşük doz aspirin (100 mgr/gün) alması önerildi. Beş yıllık izlemi sonunda hala asemptomatik olan hastanın her iki ventrikülü normal boyutlarda ve ejeksiyon fraksiyonu normal değerler içindeydi.
Left ventricular (LV) noncompaction is a congenital dysfunction of endomyocardial morphogenesis characterized by excessively prominent trabeculations and deep intratrabecular recesses of the myocardium. Its association with Ebstein’s anomaly is very rare. A 13-year-old boy presented to the outpatient clinic for a cardiac evaluation before enrolling in a sports activity. On physical examination, there was a grade 1/6 systolic murmur at the tricuspid valve location. His electrocardiogram was normal. Transthoracic echocardiography revealed numerous large trabeculations and deep intratrabecular recesses at the apex, lateral wall, and the apical part of the interventricular septum. Color flow Doppler examination confirmed the presence of blood flow within the trabeculae. The size and ejection fraction of the LV was normal (65%). The attached margins of the septal and inferior leaflets of the tricuspid valve were apically displaced and there was mild tricuspid regurgitation. The distance between the mitral and tricuspid annuli was 20 mm. The right ventricle showed no dilatation and had normal systolic function, but the right atrium was moderately enlarged. As the patient was asymptomatic and the LV systolic function was preserved, he was scheduled for regular follow-up without medication except for low-dose aspirin (100 mg/day). After five years of follow-up, he was still asymptomatic with normal size and ejection fraction of both ventricles.

Cardiovascular disease is one of the leading causes of mortality and morbidity in postmenopausal women. Menopausal changes have been shown to be related with an atherogenic lipid profile. The efficiency of statins in reducing the incidence of cardiovascular diseases has been well documented in a variety of randomized, placebo controlled trials. This review outlines the effectiveness of statins both in cardiac events and in some noticeable indications in postmenopausal women.
