A case of simultaneous anterior, inferior, and right ventricular ST-segment elevation myocardial infarction due to occlusion of the wrapped left anterior descending coronary artery

Right precordial leads ST-segment elevation (STE) predicts right ventricular infarction (RVI) in the setting of inferior myocardial infarction (MI). Such STE may also be recorded during anterior MI though it lacks any diagnostic utility over RVI that may occur in 10% of such cases. In this report we present some comments regarding physiopathology and clinical implications of right precordial leads and specifically lead V3R STE in the setting of anterior MI.
